Insulin for glycemic control in acute ischemic stroke.
نویسندگان
چکیده
Evidence-Based Answer Administering insulin to maintain glycemic control does not improve the rates of mortality or dependency, or the final neurologic outcome after an acute ischemic stroke. Additionally, in randomized controlled trials, patients in the intervention groups (those under tight glycemic control) had a 25-fold higher incidence of symptomatic hypoglycemia. (Strength of Recommendation: A, based on consistent, good-quality, patient-oriented evidence.)

Background: Hyperglycemia after acute stroke is a common finding that has been associated with an increased risk of death. For the last several years, it was believed that post-stroke hyperglycemia may worsen brain infarction in animal models. According to previous studies, the anti-inflammatory effect of insulin has a protective role on ischemic tissues. Glucose-insulin-potassium (GIK) infusio...
متن کاملEarly insulin glycemic control combined with tPA thrombolysis reduces acute brain tissue damages in a focal embolic stroke model of diabetic rats.
BACKGROUND AND PURPOSE Therapeutic effects of early insulin glycemic control for poststroke hyperglycemia in combination with tissue-type plasminogen activator (tPA) thrombolytic therapy have not yet been studied but are of great clinical interest. In this study, we tested the effects of insulin plus tPA combination in a model of focal embolic stroke in Type I diabetic rats. METHODS Streptozo...
متن کاملThe Relation between GABA and L-Arginine Levels with some Stroke Risk Factors in Acute Ischemic Stroke Patients
Changes in extra and intracellular neurotransmitter amino acids concentration in the early stage of acute cerebral ischemia have been reported. In this the study, serum level of gamma aminobutyric acid (GABA) and L-Arginine in acute ischemic stroke patients was assessed. 60 patients with acute ischemic stroke and sixthy healthy volunteers as a control group were assessed. Serum GABA was measure...
متن کاملAggressive Glucose Control for Acute Ischemic Stroke Patients by Insulin Infusion
BACKGROUND AND PURPOSE Hyperglycemia after acute ischemic stroke (AIS) is associated with poor outcomes. However, there is no consensus as to the optimal method for glycemic control. We designed an insulin infusion protocol for aggressive glucose control and investigated its efficacy and safety. METHODS We applied our protocol to patients within 48 hours after AIS or transient ischemic attack...

Background: Thrombolytic therapy is the only approved treatment for acute cerebral ischemia. The hemorrhagic transformation is the greatest complication of this treatment, which may occur after recanalization of occluded artery. The aim of this study was to determine factors associated with clinical improvement and worsening in patients with acute ischemic stroke treated with intravenous th...
